Insurance Billing

We will gladly bill your insurance for services provided in this office; however, we doneed your help. It is impossible for us to know the exact terms and conditions of every insurance policy.

  1. Please make sure we are given a copy of your most recent insurance card. Many companies change your identification number from time to time, and we must have the correct number in order for your claim to be proccessed.
  2. KNOW YOUR COVERAGE! You should find out the amount of your co-pay, deductible, and any limitations or exclusions applying to your coverage. If you have any questions about your plan, call the 800 number on your card.
  3. Copayments are required at the time of service.
  4. If you are NOT the subscriber on your plan, please provide the date of birth of the subscriber and your relationship to him or her.

We will do our best to work with you to get your insurance to make payment for services; however, YOU are the one ultimately responsible for the payment of any and all charges.
